Works under direction of Director of Activities by assisting in carrying through activity programs that are designed to meet the needs of every resident. Ability to work with staff and residents, maintain records, run entertainment programs for residents.

Availability for weekends, holidays and evenings.

SPECIFIC DUTIES:

1.                  Assures that every resident is informed of activity program:

A.                Posts monthly calendar on each floor.

B.                 Distributes copies of monthly calendar to each resident’s room.

C.                 Lists daily programs on daily board in lobby.

D.                Announces activities, prior to beginning, on the PA system.

2.                  Attends in-service programs as directed.

 
3.         Maintains appropriate records relating to program attendance and individual visits.  Records comments in resident record as directed.

Responsible for providing direct nursing care to the residents and to supervise the day-to-day nursing activities in accordance with current, federal, and local standards governing the facility. He/she is responsible for ensuring that the highest degree of quality care is maintained at all times.

Other responsibilities include:

• 7-3 , 3-11 and 11-7 shifts, every other weekend
• Delivers direct and indirect nursing care to patients/residents, fostering the maintenance of the highest functional level for the patient/resident at each stage of illness and emphasizing family inclusion. Creates and maintains an atmosphere of warmth, calm, and positivity conducive to the nursing care, treatment, rehabilitation, and/or terminal care of patients/residents
• Effectively integrates assessment data into a comprehensive nursing assessment inclusive of MDS, interim plan of care per unit standard, data from family members, and the involvement of other disciplines, as necessary
• Completes patient/residents’ initial, quarterly, and annual assessments in a timely manner and consistent with unit standards
• Ensures that nursing staff under direct supervision administer nursing care to patients/residents in accordance with all Nursing Department standards of practice
• Provides assistance and guidance to nursing staff in providing direct care to patients/residents
• Observes job performance of nursing staff and provides direct feedback. Documents and provides information to Unit Manager/Supervisor related to staff’s individual job performance
• To maintain continuity of nursing care during shift transitions, receives and provides reports (both verbal and written) to nursing staff and management and documents pertinent information on 24-hour nursing report in accordance with established procedure

Job Requirements

Graduate of an accredited Nursing program required

Must be licensed in good standing as a Nurse

Prior experience in a skilled nursing care facility, acute care setting, or other related facility required

Must be able to pass a background check

CPR/PPD or X-ray

The Certified Nurse’s Aide Job Responsibilities:

·         Care for residents in long term and short term rehab.

·         Provide assistance in activities of daily living, serving meals, feeding, bathing, ambulating, turning and positioning.

·         Maintain patient stability, recording intake and output of patients.

·         Provide comfort and care utilizing resources and materials, transporting patients, answering call lights and requests promptly, reporting observations of your patient to the nurse supervisor.

·         Documents actions by completing forms, reports, logs and records.

·         Maintains work operations by following proper policies and procedures.

·         Serves the facility and protects by adhering to professional standards, federal, state and local standards.

·         Participates in all educational requirements. Provide a warm and caring attitude toward patients and residents assigned.

·         Maintains all resident and patient information confidential.
